Core Prevention

The Core Prevention Team offers physician offices in Tennessee serving Medicare beneficiaries a variety of quality improvement services under the 9th Scope of Work contract with CMS.  We offer primary care practices expert assistance to fully utilize electronic health record (EHR) systems to enhance patient safety, office efficiency, and quality of care. Best practices are incorporated for improving rates for adult immunization (influenza and pneumococcal), breast cancer screening, and colorectal cancer screening.
  
Practices were recruited based on their having an EHR that is certified by the Certification Commission for Health Information Technology (CCHIT) in place at the time of recruitment. Once the practices have signed up to participate in this task, the team works with each of them to collect data for these four measures directly from their EHR systems. The primary goal of this task is to help practices understand the importance of improving care management processes for disease prevention, and therefore, improving patient outcomes.  Many physicians do not realize the vital role their EHRs play in helping improve the quality of patient care by tracking these measurements and using the software alerts and reminders in their systems to the fullest.

Additionally, the QIO encourages the support of Health Information Technology (HIT) and care management processes that have the potential to improve cancer screening rates and immunizations by timely notification of providers and beneficiaries when a screening should be scheduled and performed. The use of EHRs helps improve patient communication and helps providers give patients better access to timely information and improve physician office efficiency.  Participation in this project will also help prepare providers for future pay-for-performance initiatives. 

The team consisting of two Quality Improvement Specialists, one Health Information Technology Specialist, and a Manager who work closely with the recruited practices, as well as, their vendors to help them improve the quality of care they deliver in their practices. CMS and QSource both recognize the role that healthcare professionals play by providing and promoting potentially lifesaving preventing services and screenings for Medicare patients. Medicare covers preventive services more now than ever before, yet few physicians or patients realize this and take full advantage of their coverage.
