About QSource Benefits


	QSource Benefits Health Insurance QSource provides health insurance for associates, their spouses, and eligible dependents through UnitedHealthcare. QSource pays 88% of the healthcare premium; the associate is responsible for 12%. The UnitedHealthcare Choice Plus plan gives associates the freedom to see any doctor or utilize any hospital in the Choice network, including specialists, without a referral. Highlights of the coverage are as follows: No Annual deductible 100%/$15 co-payment for physician care 100%/$500 co-payment for hospital inpatient 100%/$0 co-payment for outpatient surgery 100%/$0 co-payment for diagnostic and therapeutic services $15 per visit—eye exam Routine check-ups are included Mammograms are included Childhood immunizations are provided Out-of-Network—70% after $1,000 deductible Prescription Drugs—$10/$30/$50 Life Insurance QSource provides life insurance for associates, their spouses and eligible dependents.
