Life Insurance
QSource provides life insurance for associates, their spouses
and eligible dependents. Associate: two times annual base salary,
maximum $100,000 insurance: spouse: $5,000; unmarried children
(6 months to 24 years): $2,500. Premiums are paid by QSource.
Long-term Disability
QSource provides long-term disability insurance for associates.
After 60 days, payments are made at the rate of 66-2/3% of monthly
base salary, subject to a maximum payment of $5,000 per month.
Premiums are paid by QSource.
Retirement Program
QSource offers a tax deferred annuity (403b) to associates through
the Principal Financial Group. The annuity plan is administered
by Gardner & White. Voluntary contributions may be made by
the associate during the first two years of service. After two
years, QSource automatically
contributes 4% of associate’s base salary each year, and
the associate can contribute up to an additional 6% of base salary
which will be matched at 50% by QSource.
Dental Insurance
QSource offers dental insurance for associates, their spouses,
and eligible dependents through Delta Dental. Premiums are
paid by the associate.
Spending Accounts
QSource offers two types to associates: Health Care Account and
Dependent Care Account. These accounts provide a way for associates
to pay for certain health care and dependent care expenses with
tax-free dollars.
Salary Conversion Plan
Tax regulations allow associate contributions for medical and/or
dental insurance benefits to be paid before income taxes (Federal
and State) and Social Security (FICA) taxes are deducted.
AFLAC Supplemental Insurance
Various types of supplemental insurance are available to QSource
associates. All premiums are paid by the associate but are included
in the Salary Conversion Plan. The AFLAC insurance policies are
not administered by the Human Resource Department. For insurance

Holidays
QSource observes 9 holidays that the office is closed; associates
are eligible for a bank of 80 paid holiday hours per fiscal
year: The holidays are: New Year’s Day, Memorial Day,
Fourth of July, Labor Day, Thanksgiving Day, the day after
Thanksgiving, Christmas Eve, Christmas Day and New Year’s
Eve.
Vacation
QSource full-time associates are eligible for the following annual
leave with pay:
Non-managerial associates
1-5 years = 10 days
6-10 years = 15 days
11 + years = 20 days
Managerial associates:
1-10 years = 15 days
11 + years = 20 days
Sick Leave
Leave is accrued at the rate of one day per month: 12 days per
year.
Education Assistance
QSource offers tuition reimbursement for undergraduate and graduate
courses to qualified associates (dependent upon the merits of
each individual request and subject to budgetary consideration).
Part Time Employee Benefits
Benefits are provided to part-time
associates on a pro-rated basis.
