Practice Transformation

From one-touch solutions on mobile phones to telemedicine and video conferencing, technology has changed how patients and their healthcare providers live and interact. Qsource has been on the forefront of that change, starting with assisting practices meet requirements for the integration of health information technology (HIT).

From electronic health record implementation to population health initiatives, we have helped practices coordinate patient care more efficiently and effectively. Qsource served as a Regional Extension Center and continues to provide the same level of service through our Practice Solutions division in all the states we serve. Our specialists have worked to assist with health information exchange and interoperability, and the use of practice-level data to improve patient outcomes.

We support participants in the Transforming Clinical Practice Initiative (TCPI) and Tennessee practices in the regionally based Comprehensive Primary Care+ (CPC+) program with delivery transformation and multi-payer reform for the CPC+ advanced primary care medical home (PCMH) model.
