Our focus in Nebraska is oversight of the CMS End Stage Renal Disease Program known as the Heartland Kidney Network (ESRD Network 12) that also serves Iowa, Kansas and Missouri.

We promote and support quality dialysis care and outcomes of ESRD patients and kidney transplant patients through establishing Network goals and facilitating quality improvement activities and the collection, analysis and exchange of relevant incidence, prevalence, surveillance and outcome data for performance measures. Through educational resources, and activities such as learning and action networks (LANs), we strive to be the expert resource for patients, providers and partners in the ESRD community to advance the quality of renal care.

Operating within the Qsource ESRD Network Strategies division, staff located throughout the state are supported by our office in Missouri and can be contacted for other Qsource services available to healthcare providers, patients and other stakeholders.
