Medical records may be reviewed by Qsource case reviewers at the request of the hospital for a higher weighted DRG. Upon receipt of the record, the documentation is reviewed for compliant coding, diagnostic grouping and any other potential issues to ensure the hospital’s billing/coding is supported by the medical record documentation.
