Chronic Kidney Disease (CKD)
With a strong reputation as a leader in facilitating change and collaboration, QSource implements projects and conducts case review activities to improve care across all settings. QSource is one of 10 quality improvement organizations awarded the Chronic Kidney Disease (CKD) Project, a statewide task with a focus in Shelby, Davidson, Knox and Hamilton counties.
QSource focuses on three clinical areas:
- Detection of CKD in diabetic beneficiaries
- Appropriate medication treatment (ACE inhibitors/ARBs) to slow the progression of kidney failure
- Adequate counseling prior to initiation of dialysis as evidenced by placement of an arteriovenous (AV) fistula for hemodialysis patients.
The goals of the CKD Project are to:
- Detect the incidence and decrease the progression of CKD
- Improve care among Medicare beneficiaries through provider adoption of timely and effective quality of care interventions and beneficiary education
- Foster key collaborations for system change at the state and local level
