Transformational
(Cultural) Change:
Transformational change occurs through collaboration, partnership, and commitment
to a person-directed care (PDC) approach to quality improvement. This model enhances
a residents’ quality of life by changing the culture and supporting an
environment that promotes the residents’ freedom, independence, and autonomy.
The foundation of transformational change will only occur with a positive organizational
culture that is directed and supported by the facility administration.

Culture Change begins with listening to your staff and residents.
Capturing their needs and investigating into why they may be
unhappy or have needs that you may not have been aware of,
will give them an opportunity to voice these concerns. This
begins with the survey process; we have two survey tools (created
by Primaris Healthcare) that can be used to survey both your
staff and residents to collect valuable information for your
leadership and lead clinical staff to begin to look at this
extremely important component of your nursing home.
