    Alzheimer's Awareness Month and Family Caregivers Month

    Join us this November as we commemorate Alzheimer's Awareness Month and Family Caregivers Month. In recognition of these important observances, we're pleased to share valuable resources and support for those affected by Alzheimer's and the dedicated family caregivers who provide unwavering care and support for their loved ones. Explore the comprehensive resources to empower and inform both those living with Alzheimer's and the caregivers who stand by their side.

    Diabetes Awareness Month

    November is National Diabetes Month, a time when communities across the country seek to bring attention to diabetes.

    • Download this flyer (available in English and Spanish) from the National Institute of Diabetes and Digestive and Kidney Diseases with tips to help prevent diabetes health problems.
    • This online Diabetes Resource Guide, compiled of evidence-based recommendations, tools, and resources, has been created to assist providers and other healthcare professionals who screen for those at elevated risk and manage individuals with diabetes. This resource guide contains sections with fact sheets (broken down into sections for consumers and healthcare providers), tool kits, and helpful resources.

    Immunization

    Qsource and Viven Health have partnered to provide an evidence-based, interactive, behavior change program that specifically targets the pneumonia vaccine. A crucial part of making this educational program a success will be partnering with groups who also have a passion for improving these rates and can directly reach health care providers and the general community. The combination of a proven education/behavior change program with a targeted distribution plan will make this program more likely to improve pneumonia vaccine rates. Key features of the program include:

    1. Easy to register – scanning of a QR code is all that is needed
    2. Short – it takes only minutes to complete
    3. No cost, the program is free to everyone

    Qsource and Viven Health have already piloted this program in nursing homes and one community, and it has been very well-received. We would now like to launch this program statewide and reach 1,000,000 people!

    In partnership with the Indiana Department of Health Office of Minority Health, Qsource has produced a video recorded in Spanish that highlights the impact that vaccines have had in eradicating a variety of diseases, the benefits of taking the COVID vaccination, and website links where vaccines can be accessed.
