    National Recovery Month, which started in 1989, is a national observance held every September to promote and support new evidence-based treatment and recovery practices, the nation’s strong and proud recovery community, and the dedication of service providers and communities who make recovery in all its forms possible.

    Hospital Readmission Reduction

    This How-to Guide is designed to support hospital-based teams and their community partners in implementing improved care processes to ensure that patients who have been discharged from the hospital have an ideal transition to the next setting of care (such as a primary care practice, home care, or a skilled nursing facility).

     

    This postcard, available in English and Spanish, provides guidance to help individuals determine if the doctor’s office, urgent care, or the emergency room is the most appropriate health setting for a specific healthcare need.
