Qsource Recognized for Outstanding Partnership with Tennessee Heart Health Network

Qsource staff recently received accolades for their expertise and assistance provided to the Tennessee Heart Health Network. Qsource supported the statewide system of primary care providers, health systems, community-based organizations, and academic centers for three years as they identified, developed, and implemented patient-centered approaches to improve quality of care and outcomes for Tennesseans with heart disease.
Qsource engaged primary care practices for the Network. Our Practice Solutions Team helped to implement evidence-based interventions such as pharmacist/physician collaborations, patient communications, and integrating health coaches and motivational interviewing to assist patients in reaching heart health goals.
The team facilitated open and ongoing discussions with practices providing guidance to support successful implementation of their chosen interventions using Agile and Implementation Science methodologies. As part of the Advisory Board for the Tennessee Heart Health Consortium, our team worked to move the needle on heart health in the state.
Qsource’s partnership contributed to:
- 8% improvement in clinical outcomes for hypertension control
- 4% improvement in clinical outcomes for smoking cessation
- Improved capacity for implementing change in healthcare practices
“In one specific instance, a practice reported a cost savings of over $12 million by providing the appropriate care and avoiding expensive hospital admissions and readmissions related to cardiovascular disease,” said Jennifer Ride, Qsource’s Practice Solutions Team Director.
