Data-Driven Fall Prevention in Nursing Homes: A Qsource Case Study

Falls remain one of the most significant safety risks in skilled nursing facilities. For high-acuity residents, fall prevention requires more than awareness. It takes consistent tracking, root cause analysis, staff education, adequate support systems, and documentation that reflects timely and appropriate action.

A facility participating in Qsource’s Culture of Safety Center Fall Prevention Project began with an initial fall rate of 38% and set a goal to reduce that rate to 34.2% by August 2023. Through targeted interventions, proactive risk management, and frontline staff engagement, the facility dramatically exceeded expectations, reducing its fall rate to 21.6%. Major injury falls were reduced from 2.2% to 0%.

This work was especially meaningful because the facility was also preparing for a State survey. Qsource supported the team by helping draft a comprehensive Plan of Correction, identify and document corrective interventions, and strengthen compliance with reporting and notification protocols for resident incidents such as falls and injuries.

The facility’s improvement strategy focused on practical, sustainable interventions. A fall risk tracking tool was introduced to help identify patterns related to incontinence, medications, and other contributing factors. This allowed the team to move beyond general fall prevention efforts and develop customized interventions based on each resident’s actual risks.

Staff education was also a critical part of the project. Clinical staff and agency staff received re-education on safe transfer techniques using gait belts, night rounding protocols for high-risk residents, and complete fall incident reporting. The facility also enhanced staffing capacity by hiring additional Certified Nursing Assistants and coordinating more strategically with agency partners.

The results show what can happen when data, education, staffing, documentation, and leadership support are aligned. The facility not only surpassed its fall reduction goal, but also improved staff preparedness, documentation accuracy, and survey readiness.

For nursing home leaders, this case study reinforces an important lesson: meaningful fall reduction is possible, even in high-acuity environments, when teams use data to guide decisions and support staff with the tools, training, and structure needed to respond consistently.
