Between 2021 and 2024, inpatient admissions for ESRD patients decreased by 50 percent. Access to transplant remains a critical priority in kidney care. Through focused outreach, education, and data-driven collaboration, 7,097 patients were assisted in becoming active on the kidney transplant waitlist. At the same time, modality education and support efforts led to 6,999 patients starting dialysis on a home modality of their choice and 7,390 patients transitioning to home dialysis. Preventive care efforts also showed strong results. During the 2024 to 2025 flu season, more than 80 percent of dialysis patients received the flu vaccine, helping reduce preventable complications and protect vulnerable populations.
