Letter of Interest (LOI)

LOI Deadline: Applicants must complete a short form and submit a simple Letter of Interest (LOI) in order to be considered for a Qsource Community Partnership (QCP) grant. The deadline for submitting the completed form and LOI to Qsource for the FY 2020 QCP grants funding program is July 18, 2019 11:59 PM (CST).

LOI Document and Format: On June 14, 2019, Qsource will post the QCP FY20 Application Form and LOI template on its website here. Qsource prefers to receive the applicant’s form and LOI electronically through our QCP portal, but will also provide a downloadable form and template that can be submitted by email. Applicants must complete the LOI in the template provided, (no more than 3 pages, standard 8.5 x 11” letter size), and submit it by the July 18th due date.


QCP Priorities and Funding Focus: Qsource seeks to fund projects or programs that align with Qsource’s mission. Further, Qsource places a priority on funding new projects that exhibit potential for sustainability and are designed to drive impacts in at least one of the following key priorities, to:

Address local community health needs

Serve as a catalyst for innovation

Improve one or more health outcomes

Eligibility

What We Fund: To be eligible for funding, applicants must be nonprofit organizations that are organized and operated for charitable purposes and recognized by the IRS as tax-exempt public charities under Sections 501(c)(3) of the Internal Revenue Code. Accredited public educational institutions and governmental agencies without an IRS determination letter may also be eligible to receive a QCP grant award. All applications are subject to Conflict of Interest (COI) review and approval.

FY 2019 QCP Grants Process & Key Dates

QCP’s grant application process and awards coincides with Qsource’s Fiscal Year timing. Fiscal Year (FY) 2020 is August 1, 2019 – July 31, 2020.


June 14, 2019 – Application Form and Letter of Interest Template Posted

An Application Form, Letter of Interest (LOI) Template and LOI Application Scoring Tool posted to Qsource’s website here on June 14, 2019. The Letter of Interest (LOI) will provide a general framework (in no more than 3 pages) for applicants to express their project’s concept, community need and approach to how the project will be accomplished and managed. A Letter of Interest Scoring Tool is also be posted to provide insight into how all applicants’ Letters of Interest will be evaluated.


July 18, 2019LOI Grant Applications Due Date

The deadline for grant seekers to submit the completed Application Form and LOI to Qsource for the FY 2020 QCP grants funding program is July 18, 2019 11:59 PM (CST)


August 30, 2019 – Applicants Notified of LOI Selection and Invitation to Submit Final Request for Application (RFA)

Grant seekers whose LOIs are selected will be invited to submit an expanded response via a RFA in order to detail their proposed projects more thoroughly, including specifics for project scope, goals and measurement, critical budget or timeline considerations, or community intervention models and evidence based practices to be used to ensure project success.


September 28, 2019 – Request for Application (RFA) Due Date

The deadline for grant seekers to submit a RFA response for the FY 2020 QCP grants funding program is September 28, 2019 11:59 PM (CST).


November 26, 2019 – RFA Evaluations Complete

QCP Committee completes final selection of (FY) 2020 QCQ grant awards and notifies recipients.


By December 10, 2019 – QCP Grant Awards for (FY) 2020 Announced
