Dawn M. FitzGerald has served as the Chief Executive Officer of Qsource since 2007. Prior to this, Ms. Fitzgerald held several positions at Qsource including, Chief Operating Officer and Vice President for the Disparities Reduction Program.

Qsource holds the Centers for Medicare & Medicaid Service (CMS) contract known as the Medicare Quality Improvement Network – Quality Improvement Organization (QIN-QIO) for the states of Alabama, Indiana, Kentucky, Mississippi and Tennessee, collectively known as the atom Alliance.

In addition, Qsource serves as the Tennessee’s Medicaid (TennCare) External Quality Review Organization (EQRO) and former Health Information Technology Regional Extension Center (HITREC known as TNREC). Qsource also has numerous commercial and state contracts that provide provider payment and technology adoption support, program evaluation, consultation, and quality improvement project assistance to both Tennessee and to Arkansas providers, the latter through its affiliate organization, Qsource of Arkansas.

FitzGerald has over 20 years of experience in healthcare quality measurement, quality assurance and quality improvement. She has co-authored several articles on quality improvement programs, healthcare disparities, and the analysis of Medicare data.

FitzGerald has served on the Institute of Medicine’s (IOM) Committee on Future Directions for the National Healthcare Quality and Disparities Reports; is a former member of the National Quality Forum’s (NQF) Ambulatory Measures of Health Care Disparities Workgroup and the NQF panel on Performance Measures for Minority Populations and Implementation and Improvement Workgroup; and the NQF Healthcare Disparities and Cultural Competency Consensus Standards Committee.

FitzGerald is also a member and past president of the American Healthcare Quality Association (AHQA). She has also served onn the University of Memphis, School of Public Health Dean’s Advisory Board; Healthy Shelby Governance Council; and was listed as a 2013 “Super Women to Watch” by the Memphis Business Journal and a 2014 “Healthcare Hero” awardee by the Nashville Business Journal.

FitzGerald received her Masters of Science in Developmental Psychology from the University of Florida and Masters of Business Administration from the University of Memphis.

Craig Hofer, is chief financial officer for Qsource, a nonprofit, 501(c) (3) healthcare quality improvement organization, health information technology consultancy and government contractor. As CFO, Hofer leads the work of internal shared service teams in corporate Finance, Human Resources, and Information Technology & Security for Qsource and its affiliates. He joined Qsource in 2010, bringing a focused expertise in accounting, budget planning, and cost allocation.

As CFO, Hofer is member of the organization’s senior leadership team and is specifically responsible for integrating strategic process improvements across business, financial, technology and security systems. While at Qsource, he’s helped transform operational accounting methods and programs, implemented a restructured corporate performance management system, led conversion to full CAS coverage, and improved electronic documentation of contracting and RFP business proposal systems.

Mr. Hofer holds a Bachelor of Business Administration (BBA) in Accounting from the University of Memphis. He earned his CPA license in 1985 while working for the 12th largest international accounting firm and practiced public accounting as an actively licensed CPA for 10 years.

As the vice president of operations, Cori Grant, is responsible for the strategic and operational leadership of Qsource and its services. She oversees corporate operations and support services including the Centers for Medicare & Medicaid Services (CMS) Quality Innovation Network-Quality Improvement Organization (QIN/QIO) contract, analytics, marketing and communications and process improvement.

She has a Master of Science in Epidemiology from the University of Texas School Of Public Health, a Master of Business Administration from the University of Tennessee and is due to complete a Ph.D. in Epidemiology at The University of Memphis in 2017.

Before joining Qsource, she spent seven years conducting quality improvement projects for a large patient-centered healthcare system. During that time she directed key customer and community-based research including customer experience, community image and branding studies to inform strategic planning and business development.

Passionate about improving the healthcare experience, quality of care and outcomes for patients and families, Grant has created novel means to document and monitor the patient experience. She is an active member of the Healthy Shelby End of Life Collaborative, a multi-disciplinary community-wide initiative led by the Shelby County Mayor’s office and dedicated to improving the education and coordination of healthcare at the end of life.

Ms. Archer has more than 24 years’ experience in healthcare and is a Registered Nurse (RN), and Certified Professional in Healthcare Quality (CPHQ), with a Bachelor of Science (BS) degree in Organizational Management. She has worked effectively under QI/PI contracts with the Centers for Medicare & Medicaid Services (CMS) as a leader at a federally designated QIO for 17 years, with state Department of Human Services for 15 years and Department of Health for five years.

Ms. Archer has led over fifty QI/PI and health information technology (HIT) projects under contracts at the state and national level, which focused on patient safety and clinical services provision, including emergency care, cardiac care, stroke prevention, behavioral health, care coordination, patient satisfaction, and data collection through topic-specific audit tools. In addition to these projects, other key functions of her work include formal stakeholder network assembly and development of innovative program tracking tools and communication devices to enhance patient safety and offer methods for large-scale improvement.

Her skills include the development of innovative provider education outreach and tools, utilization review methods, QI projects, and pay-for-performance (P4P) initiatives. She currently provides executive leadership, contract oversight, and project management for multiple federal and state contracts.

Ben Heavrin is a board certified emergency medicine physician who has been practicing in middle Tennessee for over a decade. He completed his undergraduate studies in economics at Princeton University, and he then attended Vanderbilt University for both medical and business school. Ben has vast experience in healthcare clinical leadership. He has served within academic medical centers, large health systems, health plans, and the public sector. Ben’s wife, Jill, is also an emergency medicine physician. They have two children, Jane and Luke. Ben is a Colorado native. He is an active golfer, runner, and outdoorsman.

EQRO Director John Couzins has a Master of Public Health in Epidemiology and is a Certified HEDIS Compliance Auditor (CHCA). His nearly 20 years’ experience in research design, methodology and statistical analysis and 16 years’ experience in quality assessment and improvement methods, Medicaid policies/processes and data systems, and managed care delivery systems, organizations and financing are bolstered by 12 years’ conducting HEDIS audits of Medicaid, commercial and Medicare health plans throughout the country. Couzins also has more than a decade in management, including review teams, contract performance and deliverables, internal data and quality control plans and strategic planning for the External Quality Review Organization (EQRO) Division at Qsource. Jennifer Ride is Qsource’s Director of Practice Solutions (previously the Health Information Technology (HIT) Regional Extension Center for the State of Tennessee). She has 25+ years’ experience in management, support and training relative to HIT, electronic medical records (EMRs) and physician practice management (PPM) software, and eight years as project manager, administrator or director.

Ride has overseen the successful conversion of numerous systems through training abilities that consistently result in increased employee performance and job satisfaction. She has a long history in the healthcare industry of increasing sales, cash flow and overall profitability. Skilled as a liaison between all members at all stages of the decision chain. In-depth experience in multiple federal compliance fields, including the Office of the National Coordinator for Health (ONC), The Centers for Medicare and Medicaid, Health Insurance Portability and Accountability Act (HIPAA), Clinical Laboratory Improvement Amendments (CLIA) and Occupational Safety and Health Administration (OSHA). Ms. Geltmaker serves as the Quality Program Director for Qsource, Kentucky’s QIN-QIO, where she provides oversight and direction for the quality improvement activities associated with CMS’ 11th Statement of Work.

Ms. Geltmaker has a passion for improving health care and more than 27 years’ in leadership, project management, business development, healthcare marketing and referral development, and patient care. Her knowledge and insight includes the day-to-day operations in settings such as the physician office, hospital, hospice, home health and outpatient rehab. In her nearly 11 years with the QIO, she has dedicated her efforts to successfully supporting teams, as well as assisting providers and stakeholders directly, with implementing quality improvement methodologies for population health, patient safety, and EHR adoption. She has conducted live and virtual presentations, facilitated learning and action network activities, and provided diabetes self-management education.

Tammy received her baccalaureate degree in nursing from Indiana University Southeast and master’s degree in health administration from Webster University. She has also received certifications as a Diabetes Empowerment Education Program (DEEP) Peer Educator, and obtained ongoing training in Cultural Competency, as well as a Legal Nurse Consultant certificate.

With advanced knowledge in regulatory compliance organizations and analytical process development, Stephanie has served on Quality Performance and Board of Directors committees for both healthcare and collegiate education. Stephanie Smith serves as the Executive Director for Qsource ESRD Network Strategies-Heartland Kidney Network (End Stage Renal Disease Network 12) for Kansas, Missouri, Iowa, and Nebraska. With primary focus on quality within dialysis facilities and transplant centers, Ms. Smith is a liaison between community stakeholders, dialysis corporations, and patient representatives. Smith is a healthcare administrator with more than 12 years’ experience in end stage renal disease, long term care management and health agency consultation. With advanced knowledge in regulatory compliance organizations and analytical process development, Stephanie has served on Quality Performance and Board of Directors committees for both healthcare and collegiate education. Her focal strengths are in risk management, assignment complexity prioritization, and high level customer focus coaching with communication effectiveness. Stephanie received her baccalaureate degree from University of Missouri and masters in Healthcare Administration from Webster University. Stephanie strives to create an environment in which her employees thrive by following quality principles and processes.
